To register by fax, address completed forms to Showcare at 1-866-268-9074 / 416-421-6226, Attn: CPA 2013 Registration. To register by mail, complete this form in full and return to Showcare, 101-2770 14th Street, Markham, ON L3R 0J1. Payment information must accompany the registration form. Payments by cheque should be made out to the Canadian Physiotherapy Association. If faxing, please do not mail your registration as well.
Refund Policy
All refund requests must be received in writing. Refund requests received on or before March 25, 2013 will be refunded in full, less a $75.00 administration fee. Refund requests received after March 25 and before April 30, 2013, will be refunded 50%. No refunds will be given after April 30, 2013. In the case of a course cancelation by the host, refunds will be applied in full. In the case of a cancelation by the registrant, refunds will apply as above. Past the final refund date, registrants may find a qualified replacement registrant for the course, at no additional fee, provided the replacement is of the same membership status as the original registrant.
